Jennifer C. Fabre, DNP, APRN, ANP/FNP-C
Chief Health Officer and Director of Quality & Clinical Risk Management
Dr. Jennifer C. Fabre has been with Teche Health since 2000 and currently serves as Chief Health Officer and Director of Quality & Clinical Risk Management. She leads the organization’s National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home recognition process. As a member of senior management, she has assisted with the planning and implementation of satellite locations, including school-based health centers and a mobile unit.
Dr. Fabre also helped implement the organization’s ambulatory electronic health record systems and serves as its provider champion. In this role, she works with vendors to improve clinical workflows, documentation, data retrieval, and reporting required for regulatory compliance and quality initiatives.
Dr. Fabre has received national recognition for her work on electronic referral processes. She has been a guest speaker for ECRI and the National Association of Community Health Centers (NACHC) and is a co-author of “Closing the Loop on Electronic Referrals: A Quality Improvement Initiative Using the Care Coordination Model,” published in *The Journal of Ambulatory Care Management* in 2020. She has also helped develop and implement performance improvement and risk management programs and contributes to the organization’s grant-writing efforts.
Dr. Fabre has represented Teche Health through state and national colorectal cancer roundtables, health center network initiatives, and NACHC’s Clinical Practice Committee.
Dr. Fabre earned her Doctor of Nursing Practice from Loyola University Chicago and her Master of Science in Nursing from the University of Louisiana at Lafayette, where she prepared as an adult nurse practitioner. She also earned a post-master’s certificate as a family nurse practitioner from the University of Massachusetts Boston.
Her professional memberships have included the American Association of Nurse Practitioners, the Louisiana Association of Nurse Practitioners, Alpha Sigma Nu Jesuit Honor Society, Sigma Theta Tau International Honor Society of Nursing, the Louisiana Primary Care Association, and NACHC.
Dr. Fabre’s professional philosophy reflects her commitment to making healthcare delivery safe, patient-focused, efficient, and equitable. She finds great satisfaction in analyzing systems of care and clinical data and collaborating with others to improve patient outcomes. She also has five years of experience as an intermittent ambulatory field surveyor for NCQA and has served as an independent grant reviewer for the Bureau of Primary Health Care.
